Job Description Job Description Starting wage is based on years
of experience General Purpose: To provide patient care through
utilization of the nursing process. The Licensed Practical Nurse
performs a variety of nursing care and administrative duties to
ensure adequate operation of the hospital in the Long-Term Care
Unit. The LPN is able to communicate effectively with all members
of the health care team and maintains a high level of
professionalism and confidentiality, which positively promotes the
image of the hospital. Essential Duties/Responsibilities: Works
with Long term care patients, able to assist with both skilled
nursing and non-skilled Medication pass, daily patient care,
answering call lights, daily accurate and timely reporting and
charting, accurate communication between other departments of the
facility, ensures respectful workplace is upheld and following
customer service based model as the facility goals. Works with
acutely ill patients, including emergency situations if the
situation arises and extra assistance is needed in this area,
within the scope of practice and licensure Works independently as
well as functioning as a team member Ensuring patient center care
if the focus of the team and helping the team stay on task Capable
of adapting to changing environment while maintaining
professionalism in the workplace Ensuring timely charting in the
EMR system is complete Ensures appropriate and timely shift change
report is completed with all appropriate team members of the
oncoming shift Ensure accurate medication pass is done and
performed in a professional manner under state licensure scope.
Communicates thoroughly and appropriately with patient family
members as their POAs. Other Duties/Responsibilities: Enhances
professional growth and development through participation in
educational programs, current literature, in-service meetings, and
workshops. Maintain quality, safety and/or infection control
standards. Perform other related duties as required to ensure the
smooth operation of the hospital. This individual is expected to
follow all policies and procedures of NCH and observe the rules and
regulations pertaining to Corporate Compliance and confidentiality
(HIPAA). This individual is expected to keep quality patient care
at the forefront of their daily operations. This individual is
expected to work as a team player at all times, and maintain
professional working relationships with fellow employees.
Supervisory Duties: Oversees CNAs on duty Job Qualifications:
Knowledge, Skills, and Ability: The ability to read, write, and
speak English is required. Additionally, the ability to read,
analyze and interpret common scientific and technical journals,
financial reports, and legal documents. Ability to respond to
common inquiries or complaints from patients, regulatory agencies,
or members of the business community. The ability to work with
mathematical concepts such as probability and statistical inference
and apply concepts such as fractions, percentages, ratios and
proportions to practical situations. Knowledge of and capability to
define problems, collect data, establish facts and draw valid
conclusions is also necessary. Excellent typing and computer skills
are also necessary. Knowledge of HIPAA regulations and Corporate
Compliance requirements. Education or Formal Training: Must be a
graduate of an accredited program of professional nursing Current
Licensure as a Practical Nurse in the State of Wyoming Current
Basic Life Support Certification to be renewed annually Experience:
One year of long-term care experience preferred Some supervisory
skills preferred but not required. Working Environment: Working in
a hospital creates a unique work environment of both
confidentiality and hospitality. The characteristics described here
are representative of those an employee encounters while performing
the essential functions of this job. This individual can expect a
moderate noise level, such as a business office or light traffic,
and moderate foot traffic. You must have the ability to hear well
enough for any type of position (e.g. normal hearing and audiogram
including normal noise and speech discrimination). Physical
Activities: Under one third of the time you may need to climb,
balance, stoop, kneel, crouch or crawl to perform your job duties.
One-third to two-thirds of the time you will stand, walk, use your
hands, or reach with your hands and arms, or sit. Over two thirds
of the time it will be necessary to talk or hear. You will
frequently bend, reach and push or pull less than 50 pounds.
Occasionally you will squat, kneel, climb on a step ladder, twist
at the waist, and rotate above the waist. Up to one-third of the
time, you must be able to lift up to 50 pounds from a twelve (12)
inch height to waist height. The ability to see with clear vision
at 20 inches or less, 20 feet or more, strong peripheral vision,
three-dimensional vision and ability to judge distances and spatial
relationships (depth perception); and the ability to adjust focus
is necessary. NCHD promotes a 7 on 7 off rotating schedule We have
