travelers - Comfortability to take MedSurg assignments Description:
Clinical Interview Unit with 36-48 beds that can flex with
semi-private rooms as needed. Looking for a PCU RN with a minimum
of one year of nursing experience. First-time travelers are
accepted, provided they have more than one year of experience.
Travelers must live 100 miles away from any facility. Patient types
include a mixed unit ranging from Med/Surg to IMC level, as well as
long-length stay patients. Ratios depend on patient acuity, with
approximately 90% Med/Surg and 10% Intermediate Care. Common
diagnoses include DKA requiring insulin drips, pre/post-surgical
patients, cellulitis, influenza, UTIs, pneumonia, COPD, CHF, renal
failure, hepatic failure, gastrointestinal disorders, soft tissue
injury and minor fractures, DVTs, and overflow from CVA patients.
Responsibilities include administering oral, IV, IM, and SQ
medications, titratable vasoactive drips, TPN/tube feeding, and
blood product transfusions. Manage peripheral IV, midline, PICC and
central line access, chest tubes, nasogastric tubes, drains,
urinary and fecal collection devices. Provide preventative and
restorative skin and wound care. Telemetry monitoring performed by
nurses who read and interpret cardiac strips. Patient Ratios: -
Long length of stay (MedSurg/No Tele): 1:5-6, with 10-12 patients
per pod. - Med Surg/Telemetry (No IMC drips): 1:5, with 10-12
patients in two pods. - Intermediate Care: 1:4, with 8 patients in
one pod that may include Med/Tele patients vs. IMC patients.
Required skills include previous experience with the listed
patients, ability to read and interpret cardiac strips, wound care,
line management, drain/tube management, and drip titration of
medications. Titration experience is required with Heparin,
Insulin, Cardizem, Amiodarone, Levophed, and occasionally esmolol
drips or other beta-blockers or antihypertensives. Support on the
Unit includes the presence of CNAs with a ratio of 1:8 and charge
nurses who do not take a patient load. Phlebotomy services are
available. Respiratory Therapy is present, along with Hospitalist
services. Charting systems used include EPIC, Omnicell, and Alaris.
Potential for floating to any Adult Inpatient Unit, including Med
Surg, Ortho/Neuro/Surgical, Heart and Vascular Unit, Oncology, and
Observation, will be in rotation with other travelers. Scheduling
involves six-week schedules, with the potential for up to four
weekend shifts within that period, including Friday, Saturday, and
Sunday. There is no mandatory call time. Holiday expectations
indicate that requests for time off around winter holidays require
manager approval and are subject to blackout dates around specific
events. Other notes indicate that scrub color is navy blue and that
candidates may be expected to precept students. Audits are
performed monthly for pre/post pain scoring, requiring compliance.
