Job Description:

Custer Care & Rehab is seeking a caring individual to assist residents with their transition of moving into a skilled nursing facility as well as helping them with the social aspects of living in a long term care facility. Functions include but are not limited to: Care plan creation, work with residents and families, social isolation, room choice and personal preferences with their room, admission/referral process management. Best candidate is organized, get along well with others and has a passion to make a difference in the lives of our elderly so if you are someone that wants to make a difference in people's lives then you will want to apply for this great opportunity!
