Scheduled Weekly Hours
The Patient Navigator works directly with adolescent, adult, and
geriatric patients undergoing active treatment in an outpatient
setting. The Patient Navigator collaborates with the healthcare
team in the management of biopsychosocial-financial resources to
achieve optimal outcomes for the patient. The Patient Navigator
will work with patients and family members through their active,
palliative, and end of life care in accordance with accepted
hospital standards and policy. The Patient Navigator consistently
employs patient and family-centered care to promote safety,
quality, and satisfaction.
?Builds awareness in the patient of all available services,
assists in clarifying choices and ensures unrestricted access to
?Coordinates patient information with community physicians and
customizes patient schedules.
?Initiates meetings and/or makes arrangements with support
departments to meet the needs of patients; finance, counseling,
services/child issues, etc.
?Initiates meetings/communications with physician offices or
related professional personnel to build or maintain communication
awareness and/or address processes for the patient navigator
?Organizes and creates a plan customized for each patient
referred; scheduling, supportive resources (i.e. counseling,
services), and post-treatment resources (i.e. support
?Participates with multidisciplinary team meetings discussing
future and present patient cases and collaborates efforts to meet
?The Navigator coordinates data, community physicians, and
Regional Health resources into a seamless model of access and
benefiting patients, participating physicians, and family
?Works with staff, administration, and physicians to match
resources with patient needs.

Certification - Registered Nurse (RN) - South Dakota Board of
Education - Bachelors degree in Nursing
Experience - 1+ years of Case Management Experience; 1+ years of
Patient Care Experience
Light work - Exerting up to 20 pounds of force occasionally,
and/or up to 10 pounds of force frequently, and/or a negligible
