Job Description:
Wound Care Nurse Practitioner / Physician Assistant
(NP/PA) Position Summary The Wound Care Nurse Practitioner or Physician Assistant
provides specialized clinical care for patients with acute and
chronic wounds. This role focuses on assessment, diagnosis, and
management of complex wounds using evidence-based practices,
working collaboratively with interdisciplinary teams to promote
wound healing, prevent complications, and improve patient
outcomes. Evaluate and manage patients with acute and chronic wounds
including pressure injuries, diabetic ulcers, venous stasis ulcers,
arterial ulcers, surgical wounds, and traumatic wounds Perform comprehensive wound assessments and develop
individualized treatment plans Perform wound care procedures consistent with training and
privileges, including debridement, dressing selection, and
application of advanced wound therapies Monitor wound healing progress and adjust treatment plans as
needed Manage wound-related infections and coordinate care for limb
preservation when applicable Collaborate with physicians, nursing staff, podiatry, vascular
surgery, infectious disease, and rehabilitation services Educate patients and caregivers on wound care management,
prevention strategies, and lifestyle modifications Coordinate care transitions across inpatient, outpatient, home
health, and long-term care settings Maintain accurate and timely clinical documentation Participate in quality improvement, patient safety, and outcomes
initiatives Support compliance with wound care protocols, standards, and
regulatory requirements Comply with all regulatory, accreditation, and professional
standards Graduate of an accredited Nurse Practitioner or Physician
Assistant program Current NP or PA certification and state licensure Eligibility for credentialing and privileging DEA registration or eligibility Prior experience or certification in wound care preferred Expertise in wound assessment and management Knowledge of advanced wound care therapies and products Strong clinical judgment and decision-making skills Excellent communication and patient education abilities Ability to work collaboratively within multidisciplinary
teams Commitment to evidence-based practice and patient-centered
care Hospital inpatient units, outpatient wound care centers,
long-term care facilities, and home-based settings Schedule may include clinic hours and limited on-call
responsibilities depending on program needs Ability to stand, bend, and move throughout clinical
environments Manual dexterity for wound care procedures Visual and auditory acuity sufficient for wound assessment and
patient communication
