Job Description:
Good Samaritan Health Center seeks a dedicated Licensed
Practical Nurse (LPN) to join our Long-Term Care (LTC) team,
serving as a Local Traveler to support our mission of providing
exceptional healthcare services to underserved communities. This
role involves traveling locally between multiple facilities to
deliver high-quality, holistic care to elderly or disabled
patients, focusing on promoting wellness and improving health
outcomes. Travel between local healthcare facilities to provide
continuous care to patients in long-term care settings. Monitor
patient health and maintain records of vital statistics and medical
information. Administer medications, monitor effects, and maintain
detailed records in compliance with healthcare regulations.
Coordinate with healthcare professionals to implement comprehensive
care plans for patients. Educate patients and their families on
health maintenance and disease prevention. Assist with diagnostic
tests and medical procedures as needed. Ensure a compassionate
environment, providing emotional and psychological support to
patients and families. Proficiency in administering medications and
managing healthcare equipment. Strong organizational and
communication skills. Ability to work effectively in diverse
environments. Competency in electronic health record (EHR) systems.
