Previous discharge planning and acute care case management
experience. - Experience in progression of care. - Experience
meeting with families and discussing plans of care. Description:
Travelers must live more than 100 miles away from any facility.
Permanent staff must wait one full year to return as travelers.
Case Managers provide care for patients in all acute care
departments. The RN Case Manager will likely remain in the same
area during their contract, pending new hires. Typically, travelers
are not required to float to multiple areas during their contract.
Patient ratios are 1 Case Manager to 16-20 patients. Support on the
unit includes Case Management Assistants, Lead Case Managers,
Social Workers, and Utilization Review RNs. The charting system
used is EPIC. Weekend rotation includes working two weekends per
13-week assignment. Typical staffing support during weekend shifts
includes three RN Case Managers, one Social Worker, one Lead Case
Manager, one Case Manager Assistant, and one Utilization Review
Nurse. Travelers will not be on call. Holiday expectations include
working two of four major holidays. Time off requests may be
approved for up to seven days, with extensions required for any
full week off. Scrub color is navy blue or professional attire. 5
