patient care Description: A Physical Therapist is needed to support
the treatment of illness and injury through physical exercises and
therapies. The role assists in the rehabilitation and restoration
of body functions to promote maximum client wellness and
independence. You will provide therapy programs for patients of all
ages with developmental deficits, physical injury, or illness.
Additional responsibilities may include other duties as assigned
based on the facility’s needs. Responsibilities include
coordinating physical therapy services and providing direct
therapeutic care to assigned clients. You will teach rehabilitation
skills to clients and caregivers to enhance independent
functionality and safety. Perform preventive and rehabilitative
procedures that are individualized according to client needs and
age-specific requirements. Duties must be performed within the
scope of physical therapy practice guidelines and in accordance
with productivity standards. You will develop and revise plans of
care based on initial assessments and continual reassessment of
client and family needs. Plans should consider client history,
diagnosis, physician orders, expected outcomes, and client goals,
including discharge plans when appropriate. You will monitor the
application of treatments involving physical agents, using
equipment such as whirlpool baths, electrical muscle stimulation,
and ultrasound. Ensure compliance with utilization guidelines and
protocols. Formulate and implement appropriate teaching plans, and
communicate significant clinical findings and changes to physicians
and other personnel. Documentation of all skilled and medically
necessary care must be done thoroughly, timely, and accurately.
Plans of care and documentation should comply with Medicare, State,
Federal, and payer requirements as well as local policy.
Maintaining client and organizational confidentiality is crucial.
You will participate in Quality Management programs and complete
required in-service programs to meet standards and keep skills
current. Conduct supervisory visits as applicable, and prepare and
update written instructions for aides according to policy. You may
supervise Physical Therapy Assistants and Home Health Aides as
needed. Participation in staff meetings, case conferences, and
business development meetings is required. Deliver a high level of
customer service to all internal and external customers.
Demonstrate leadership competencies through communication,
relationship building, team development, and teamwork. Requirements
