  • Job Details
  • Fall River Health Services is looking for an LPN for multidisciplinary care team. In this role you will provide general nursing care to residents.
  • The LPN contributes to overall resident care which is guided by the resident care plan, facility policies and procedures, and professional standards of practice.
  • Responsibilities include supervising Nursing Assistants, coordinating care of assigned residents, assisting in the interpretation and implementation of facility policies.
  • Provides care within the boundaries of physician orders, department procedures and policies, and LPN scope of practice.
  • Contributes to the nursing assessment, participates in the development of the nursing diagnosis, care planning, and implementation of nursing interventions and evaluates responses.
  • Ensures resident medical needs are met, including making provisions for outside physician visits and performing teaching to residents, families, and staff.
  • Prepares, administers, and charts routine and emergency medication and treatment as ordered.
  • Ensures clinical record reflects accurate and timely documentation.
  • Performs safety and infection control procedures.
  • Participates in Quality Management program within the facility, functioning as the primary contact person on assigned shift, and supervises resident assistants to ensure compliance with plan of care.
    • Job Requirements
    • Must currently hold an active unencumbered license within the State of South Dakota.
    • OR Have a temporary permit allowing you to practice in the State of South Dakota.
    • OR Have applied for a South Dakota LPN licensure by endorsement.
    • Education commensurate with requirements of licensure.
